Dr Tehseen Khan has been appointed clinical lead for Neighbourhoods in City and Hackney, an area with diverse health and care system.

He was a Primary Care Network (PCN) clinical director, who served in community immunisation programmes.


Khan will lead the healthcare development of City and Hackney’s eight neighbourhoods.

City and Hackney has a population with complex health needs and its patient list is currently 353,000.

As part of the government’s 10-year plan, the NHS has introduced neighbourhood-based care programmes to help people avail care at their doorsteps.

Khan aims to bring a fresh approach in neighbourhood healthcare via the collaboration with communities and residents.

This includes investing in community health services, supporting self-management, and working closely with multidisciplinary teams to deliver care as close to home as possible.

He said, “This includes shifting from reactive healthcare to a proactive, prevention-focused approach, in keeping with national priorities.

“By using population health data to identify people at risk earlier, we can intervene before complex conditions require urgent care, reducing pressures on hospitals and emergency services,” he said.

“This includes investing in community health services, supporting self-management, and working closely with multidisciplinary teams to deliver care as close to home as possible,” he added.

Khan considers improved investment, workforce and effective partnerships with appropriate organisations as the solutions to break down the existing barriers in City and Hackney.

From now, City and Hackney will focus on prevention and community-based under his leadership.

He felt that technological integration, using various digital tools, reduces the burden of healthcare workers.
