A new study reveals that the physical characteristics of pills make children more or less willing to swallow them.


Researchers found that a pill’s size and shape, rather than its colour, are the strongest factors influencing whether children perceive it as difficult to swallow.

Children consistently rated elongated tablets as harder to swallow than round pills of the same volume.

The small study involved 50 children between the ages of 8 and 15 years. They were shown photographs of commonly prescribed pills and asked to rate how difficult they thought each one would be to swallow, on a five-point scale.

Those with no prior experience in swallowing pills considered every pill more difficult to swallow than those who had not.

But all participants shared similar preferences regarding pill shape.

Pill colour had no meaningful effect on children’s perceptions.

According to the researchers, understanding children’s visual perceptions of medication is important in improving treatment success and in encouraging pharmaceutical manufacturers to consider not only how medicines work pharmacologically, but also how they are experienced by the children expected to take them.

Difficulty swallowing solid medicines may lead families to seek liquid versions or to crush tablets – practices that can compromise dosing accuracy, stability, or effectiveness for some drugs.

The study, published in the Journal of Child Health Care was carried out by the researchers from Kaplan Medical Center of Israel’s largest health organisation Clalit Health Services.

Lead researcher Dr Yael Shapira Galitz commented that more attention could be paid to these factors when formulating medicines.

Understanding how children perceive different pills can help parents work with pharmacists to identify equivalent alternatives when available. It also sends a clear message to pharmaceutical manufacturers that designing child-friendly medications should include physical appearance, not only chemical formulation,” she said.

Clinical pharmacist Dotan Shaniv, also involved in the study, noted that most medicines are made as solid dosage forms because they offer precise dosing, longer shelf life and lower cost, but their appearance can deter children.
